About the memorial: Two brass plaques on oak backboards positioned one above the other, the lower plaque being a continuation of the list begun on the upper plaque. The upper one, in brushed brass, has a representation of the Burma Star etched at the top, with the title and two columns of names below. The lower one, of polished brass, has three columns of names inscribed above the Kohima Epitaph.The inscriptions are incised and painted black. The names are those of the Members of the Southport Branch of the Burma Star Association, which was founded around 1973, and were added upon their deaths. The last name is that of the last surviving member, Peter Proctor, who died in September 2019. The second name on the lower plaque, Canon Arthur Thompson, is that of a former Padre in Burma, who was Vicar of All Saints from 1962 to 1982.

The Pre-application enquiry form is a series of questions to see if your project is eligible. If it is, you will need to provide further details and submit current colour photographs of the war memorial in either a png, gif, jpg or jpeg format.
