About the memorial: Tree & plaque 16-17 December – 12 Squadron - Took off from Wickenby 1614. On return crashed 2345 at Hainton 9 miles WSW of Louth, Lincolnshire, Sgt, Whitley was critically injured and died shortly after being admitted to Louth Hospital

The Pre-application enquiry form is a series of questions to see if your project is eligible. If it is, you will need to provide further details and submit current colour photographs of the war memorial in either a png, gif, jpg or jpeg format.
