About the memorial: Proposed by John Thomas and Sarah Markham to provide and place a memorial tablet on the south wall of the parish church of Silk Willoughby in memory of their son, that the said tablet is to be of Brass and about ten and a half inch by about seven and a half inch [170 x 245 x 3] and is to bear the following inscription: Facultty Book 13, page 382 dated 26-11-1919
