About the memorial: Vertical rectangular headstone with pyramidal top dropping to Ogee shoulders, stone narrows below shoulders before widening again toward base, stands on horizontal chamfered foundation. Recessed trefoil panel toward top of stone has carved flower decoration within, dedication below in faded black lead lettering, partially missing.

The Pre-application enquiry form is a series of questions to see if your project is eligible. If it is, you will need to provide further details and submit current colour photographs of the war memorial in either a png, gif, jpg or jpeg format.

Refers to: - Thomas Burtwistle. Private 2793. 5th Battalion, Lincolnshire Regiment. Killed in action 30th June 1915. Birth recorded 1884 Hook, YKS. Present 1891 Hook, YKS with parents George and Harriet A Birtwistle (as census) and present Scunthorpe, Lincs 1911 with wife Drusilla Burtwistle (as census) (nee Littlewood). Enlisted Grimsby and resident Scunthorpe, Lincs. Missing and commemorated Ypres (Menin Gate) Memorial. Panel 21 (although note stone has ‘buried Sanctuary Wood’).
