About the memorial: Alabaster tablet in hardwood frame that hung inside the entrance of the building, destroyed in a fire on 1 December 1990. It had been originally in the Masonic Hall at Gower Street, Derby until that was demolished in 1972 as part of a redevelopment scheme. Replaced by WMO 265214.

The Pre-application enquiry form is a series of questions to see if your project is eligible. If it is, you will need to provide further details and submit current colour photographs of the war memorial in either a png, gif, jpg or jpeg format.
