About the memorial: Portable Holy Communion set, comprising flagon, 115 mm high with a stopper in the form of a St. Cuthbert’s cross, plus chalice 95 mm high and paten, 90 mm diameter, set into a velvet lined purpose built box. Each piece bears a part of the dedication. In the case of the paten, this is on the underside of the rim. All lettering is in Roman capitals.

The Pre-application enquiry form is a series of questions to see if your project is eligible. If it is, you will need to provide further details and submit current colour photographs of the war memorial in either a png, gif, jpg or jpeg format.
