About the memorial: Wooden triptych with ornately carved triangular pediment, within which is a gold-painted chalice in half-relief. Beneath is an inner central panel, containing a gold crucifix with the figures of Mary and St John at its foot, and the names of the Fallen. The outside panels of the hinged doors bear the dedication in gold-painted lettering; the inside panels of the doors record the Roll of Honour of those who served. The protruding base has a central circular motif flanked by scrolled leaves.

The Pre-application enquiry form is a series of questions to see if your project is eligible. If it is, you will need to provide further details and submit current colour photographs of the war memorial in either a png, gif, jpg or jpeg format.
