About the memorial: Tablet. It was originally at the Monkwearmouth Miners' Hall, Roker Avenue and has been moved a number of times since. In the Welfare Ground it is just inside the grounds to the left of the entrance. The tablet had become to be a very poor condition, and was replaced in 2008 by a new memorial-WMO/283301 at which time this tablet was broken up.

The Pre-application enquiry form is a series of questions to see if your project is eligible. If it is, you will need to provide further details and submit current colour photographs of the war memorial in either a png, gif, jpg or jpeg format.
