About the memorial: Formerly a two-storey brick and tile community hospital in its own grounds, but now demolished and replaced by a single-storey brick and tile medical centre. To the right of the new entrance is an oblong grey tablet with chamferred edges and bearing an incised inscription in upright sentence case roman lettering coloured black. An oblong white marble tablet in the original entrance, which later became a corridor, has now been relocated to an external boundary wall. It has a deep cornice at the head, probably intended for the placement of tributes, two brackets below, a plain narrow incised border coloured black and an incised inscription in upright capital block lettering also coloured black although much of the colour has already been lost. The entrance porch to the new building has, on its southeast wall, a glazed wooden case containing four plaques, of varying metals and styles, commemorating several events relating to the original hospital. The medical centre is on the northeastern side of the road on the site of the former hospital.

February 1919: A public meeting in Heanor decided to join with Langley Mill in providing a cottage hospital as a memorial to the men of the district who fell in the war.;1920s: Land for the hospital was donated by Mr E Miller Mundy of Shipley Hall.; 1920s: The hospital was designed by Mr H T Sudbury, architect, of Ilkeston.; 
4 July 1924: The Heanor Memorial Hospital Committee decided to postpone building work because of rising costs.; 18 March 1925: The Heanor Memorial Hospital Committee decided to accept the tender of Mr W. Wood, builder of Heanor, for £3,875.; 16 May 1925: The foundation stone was laid by Lady Smith of Dunstead; 1920s: Fundraising events and donations continued to benefit the hospital.; 1948: The hospital was incorporated into the National Health Service.; 10 August,1932: A legacy was bequeathed to the hospital by the late Miss E. Shelswell, of Langley Mill.; 2 December, 1977: A toilet annexe, a medical records office and other improvements were provided by the League of Friends.; 1993: The funds to provide for an extension, The Hallam Suite and refurbishment of the ward bathrooms and toilets were provided by the League of Friends.; 6 October 1993: A plaque marking the extension and upgrading of the hospital was unveiled by Mrs P. Zadora, chairman of the then community health service trust.; September 2013: Asbestos was discovered in the heating system during an inspection and the hospital was closed pending further investigations.; October 2014: The decision was taken to demolish the hospital.; 14 September 2015: Demolition of the hospital began. Artifacts and memorabilia were salvaged for relocation in the replacement health centre.; 2016-17: The new health centre was designed by Frank Shaw Associates, Collins Hall Green and EP Consulting, and was constructed by A&S Enterprises Ltd of Needwood, Burton-on-Trent. The total cost was about £3,500,000.; 24November, 2017: The new centre was handed over to the health authority.; 11 December, 2017: The health centre opened for patients.; 13 February, 2018: The new health centre was officially opened by Prem Singh, Chairman of Derbyshire Community Health Services NHS Foundation Trust
